Transcutaneous lower blepharoplasty with fat reposition.

Structural observations

  1. Tear trough depression

  2. Lower eyelid fat prolapse

  3. Early lid–cheek junction descent

    and Shadow formation beneath the eyes

Surgical Approach


  • Transcutaneous lower blepharoplasty

  • Tear trough release

  • Fat reposition to the hollow segment

  • Conservative skin excision

  • Preservation of lower eyelid support

Postoperative Result


Postoperative observation

  • Reduced lower eyelid prominence

  • Improved lid–cheek continuity

  • Softer shadow transition beneath the eyes

  • Rested appearance with natural expression preserved
